เซลล์เพซเมกเกอร์ (อังกฤษ: pacemaker cells, cardiac pacemaker) เป็นเซลล์ชนิดหนึ่งซึ่งมีหน้าที่ควบคุมจังหวะการเต้นของหัวใจ พบได้ที่เนื้อเยื่อหัวใจ (heart tissue) โดยสามารถเกิด depolarization หรือสร้างกระแสประสาท (action potential) ได้เอง แบ่งเป็น 2 ชนิด ได้แก่ sinoatrial node (SA node) และ atrioventricular node (AV junction)
Sinoatrial node หรือ SA node เป็นกลุ่มของเซลล์ที่อยู่บนผนังหัวใจของหัวใจห้องบนขวา ใกล้กับทางเข้าของหลอดเลือดดำเวนาคาวาบน (superior vena cava) เป็นเซลล์เพซเมกเกอร์ที่อยู่สูงที่สุด และจัดเป็นเซลล์เพซเมกเกอร์ที่ 1 (primary pacemaker) ซึ่งสามารถสร้างกระแสประสาทในภาวะปกติได้ประมาณ 70-80 ครั้งต่อนาที
Atrioventricular node Antrioventricular node หรือ AV junction พบได้ที่บริเวณระหว่างหัวใจห้องบนซ้าย (left atrium) และหัวใจห้องล่างขวา (right ventricle) ภายในผนังกั้นห้องหัวใจ (atrial septum) หรืออยู่ถัดลงมาจาก SA node โดยจัดเป็นเซลล์เพซเมกเกอร์ที่ 2 (secondary pacemaker) มีความเร็วในการสร้างกระแสประสาทรองจาก SA node คือ 40-60 ครั้งต่อนาที
อัตราในการเกิด pacemaker อัตราในการเกิด pacemaker (pacemaking rate) นั้นจะขึ้นอยู่กับเซลล์ที่มีความเร็วในการสร้างกระแสประสาทได้สูงสุด เพราะฉะนั้น ในภาวะปกติ SA node ซึ่งเป็นเซลล์ที่สามารถสร้างกระแสประสาทได้เร็วที่สุด จึงเป็นตัวกำหนด pacemaking rate แต่ถ้า SA node มีความผิดปกติ ไม่สามารถสร้างกระแสประสาทได้ เซลล์ที่มีความเร็วในการสร้างกระแสประสาทรองลงมาหรือ AV junction ก็จะทำหน้าที่แทน
