ความดันโลหิตสูงไม่ทราบสาเหตุ (อังกฤษ: Essential Hypertension) หรือ ความดันโลหิตสูงปฐมภูมิ (อังกฤษ: Primary Hypertension) เป็นความดันโลหิตสูงที่ไม่สามารถระบุสาเหตุได้ชัดเจน เป็นความดันโลหิตสูงชนิดที่พบได้บ่อยที่สุด ราวร้อยละ 95 ของผู้ป่วยความดันโลหิตสูงทั้งหมด มีแนวโน้มที่จะเป็นกรรมพันธุ์ และน่าจะเป็นผลจากความสัมพันธ์ซับซ้อนระหว่างพันธุกรรมและสิ่งแวดล้อม ความชุกของความดันโลหิตสูงไม่ทราบสาเหตุเพิ่มขึ้นตามอายุ และผู้ป่วยที่มีความดันเลือดสูงเมื่ออายุน้อยจะมีความเสี่ยงมากขึ้นที่จะเป็นโรคความดันโลหิตสูงเมื่ออายุมากขึ้น ความดันโลหิตสูงเพิ่มความเสี่ยงของโรคสมอง หัวใจ และไตตามมา

การจำแนกความรุนแรง

การจำแนกความรุนแรงของความดันเลือดออกในปัจจุบันเป็นความดันเลือดปกติ ก่อนความดันโลหิตสูง (prehypertension) ความดันโลหิตสูงระยะที่ 1 และ 2 และความดันโลหิตเฉพาะช่วงหัวใจบีบสูง (isolated systolic hypertension) ที่มักพบได้บ่อยในผู้สูงอายุ การอ่านค่าความดันใช้ค่าเฉลี่ยของการวัดความดันโลหิตในท่านั่งที่สบายและควรวัดอย่างน้อย 2 ครั้งที่มาตรวจในระยะเวลาที่ห่างกัน ในผู้ที่อายุมากกว่า 50 ปี จะวินิจฉัยภาวะความดันโลหิตสูงเมื่อมีความดันเลือดมากกว่า 140 มิลลิเมตรปรอทในช่วงหัวใจบีบ และ 90 มิลลิเมตรปรอทในช่วงหัวใจคลาย ผู้ป่วยที่มีโรคเบาหวานชนิดที่ 1 หรือ 2 หรือโรคไตและมีความดันโลหิตมากกว่า 130/80 มิลลิเมตรปรอทควรได้รับการรักษา
