Commotio cordis

Commotio cordis เป็นภาวะที่เกิดขึ้นเมื่อมีการกระแทกบริเวณหน้าอก ซี่โครง หรือหัวใจ ทำให้เกิด ventricular fibrillation จนอาจเสียชีวิตได้อย่างเฉียบพลัน คำว่า commotio cordis มาจากภาษาละติน แปลว่าภาวะไม่สงบของหัวใจ (agitation of the heart) คำนี้มีใช้ครั้งแรกในศตวรรษที่ 19 แต่ก็มีการกล่าวถึงภาวะเช่นนี้นับย้อนไปได้ถึงศิลปะการต่อสู้จีนโบราณ Dim Mak หรือการสกัดจุดตาย ซึ่งอ้างว่าสามารถทำร้ายคู่ต่อสู้ให้ถึงชีวิตได้โดยโจมตีไปที่หน้าอกด้านซ้าย ภาวะ commotio cordis แตกต่างจาก cardiac contusion หรือการชอกช้ำของหัวใจที่เกิดจากการกระแทกอย่างรุนแรงจนเกิดความเสียหายต่อกล้ามเนื้อหัวใจและผนังหน้าอก

อุบัติการณ์

commotio cordis พบได้น้อยมาก อย่างไรก็ดีมักนึกถึงภาวะในผู้ที่มีร่างกายแข็งแรงระดับนักกีฬาแล้วเกิดหัวใจหยุดเต้นเสียชีวิตกะทันหัน กีฬาที่มีความเสี่ยงที่จะเกิดการบาดเจ็บที่ทำให้เกิด commotio cordis ได้ง่ายเช่น เบสบอล อเมริกันฟุตบอล ฮอคกี้ โปโล รักบี้ คริคเก็ต ซอฟท์บอล เปโลตา ฟันดาบ ลาครอส ชกมวย คาราเต้ กังฟู รวมถึงกีฬาศิลปะป้องกันตัวอื่นๆ
