ภาวะองคชาตแข็งค้าง (อังกฤษ : priapism ) เป็นภาวะทางการแพทย์ที่อันตรายอย่างหนึ่งซึ่งองคชาต หรือคลิตอริส ได้แข็งตัวขึ้นมาและไม่คลายตัวกลับสู่ภาวะปกติโดยไม่มีสิ่งกระตุ้นทางกายภาพหรือจิตใจเป็นเวลา 4 ชั่วโมง ถือเป็นภาวะฉุกเฉินทางการแพทย์ ซึ่งต้องได้รับการรักษาอย่างเหมาะสม
