ในทางการแพทย์ กลุ่มอาการตอบสนองต่อการอักเสบทั่วร่างกาย (systemic inflammatory response syndrome) หรือ SIRS เป็นภาวะการอักเสบของทั้งร่างกายโดยไม่ระบุสาเหตุของการติดเชื้อ นับเป็นภาวะฉุกเฉินทางการแพทย์

นิยามของ SIRS

เกณฑ์ของกลุ่มอาการตอบสนองต่อการอักเสบทั่วร่างกายกำหนดขึ้นในปี ค.ศ. 1992 ซึ่งแพทย์จะจัดว่าผู้ป่วยมีกลุ่มอาการตอบสนองต่อการอักเสบทั่วร่างกายต่อเมื่อมีอาการเข้าเกณฑ์ดังกล่าวอย่างน้อย 2 ข้อ

  • อัตราหัวใจเต้น มากกว่า 90 ครั้งต่อนาที
  • อุณหภูมิร่างกายน้อยกว่า 36 หรือมากกว่า 38 องศาเซลเซียส
  • อัตราหายใจเร็ว (tachypnea) คือมากกว่า 20 ครั้งต่อนาที หรือมี PaCO2 น้อยกว่า 4.3 kPa (32 mm Hg) ในการวัด blood gas

ความแตกต่างระหว่าง SIRS และภาวะพิษเหตุติดเชื้อ

มีคำที่มักจะใช้สับสนระหว่างกลุ่มอาการตอบสนองต่อการอักเสบทั่วร่างกาย (SIRS) และภาวะพิษเหตุติดเชื้อ (sepsis) SIRS ซึ่งมีสาเหตุมาจากการติดเชื้อที่สงสัยหรือที่ทราบได้นั้นคือภาวะพิษเหตุติดเชื้อ

หรือกล่าวโดยสรุปว่า SIRS + การติดเชื้อ = ภาวะพิษเหตุติดเชื้อ

ภาวะแทรกซ้อนของ SIRS

  • ภาวะพิษเหตุติดเชื้อที่รุนแรง คือภาวะพิษเหตุติดเชื้อที่มีอวัยวะล้มเหลวตั้งแต่ 1 อวัยวะขึ้นไป
  • กลุ่มอาการตอบสนองต่อการอักเสบทั่วร่างกายทำให้เกิดกลุ่มอาการการทำหน้าที่ผิดปกติของหลายอวัยวะ (multiple organ dysfunction syndrome)
  • ความดันต่ำ (hypotension) เกิดจากการขยายตัวของหลอดเลือด
  • ช็อกเหตุปริมาตรเลือดน้อย (hypovolemic shock)
