สารกันเลือดเป็นลิ่ม (อังกฤษ: Anticoagulant, blood thinner) เป็นสารเคมีที่ใช้ป้องกันหรือลดการจับลิ่มของเลือด เป็นการยืดระยะการแข็งตัวของเลือด บางชนิดมีอยู่โดยธรรมชาติในสัตว์กินเลือดเป็นอาหาร เช่นปลิงและยุง ซึ่งช่วยให้เลือดไม่แข็งตัวนานพอที่สัตว์จะได้เลือดเป็นอาหาร เมื่อใช้เป็นยา ก็จะใช้ในการบำบัดโรคเกี่ยวกับหลอดเลือดมีลิ่มเลือด มียาเป็นเม็ด โดยยังมีชนิดที่ให้ทางเส้นเลือดดำซึ่งใช้ในโรงพยาบาลด้วย สารยังใช้กับอุปกรณ์การแพทย์ต่าง ๆ รวมทั้งหลอดเก็บตัวอย่าง ถุงถ่ายเลือด เครื่องนวดหัวใจ-ผายปอด และเครื่องล้างไต ยากันเลือดเป็นลิ่มชนิดหนึ่งคือ วาร์ฟาริน ตอนแรกอนุมัติให้ใช้เป็นยาเบื่อหนู

ยากันเลือดเป็นลิ่มสัมพันธ์กับยาต้านเกล็ดเลือด (antiplatelet) และยาสลายลิ่มเลือด (thrombolytic) เพราะเข้าไปจัดแจงวิถีทางเคมีของการจับลิ่มเลือด โดยเฉพาะก็คือ ยาต้านเกล็ดเลือดยับยั้งการจับก้อนของเกล็ดเลือด เทียบกับยากันเลือดเป็นลิ่มที่ยับยั้งวิถีทางเมแทบอลิซึมโดยเฉพาะ ๆ ของ coagulation cascade (ดูรูป) โดยออกฤทธิ์หลังจากที่เกล็ดเลือดจับก้อนในเบื้องต้น แต่ก่อนการสร้าง fibrin และผลิตภัณฑ์เกล็ดเลือดที่จับก้อนอย่างเสถียร
