วิทยาการระบาดของโรคซึมเศร้า (อังกฤษ: epidemiology of depression) เป็นเรื่องที่ได้ศึกษาแล้วทั่วโลก โรคซึมเศร้าเป็นเหตุความเจ็บป่วยสำคัญทั่วโลก ดังที่วิทยาการระบาดได้แสดงแล้วความชุกชั่วชีวิตต่างกันมาก เริ่มตั้งแต่ 3% ในประเทศญี่ปุ่น และ 17% ในสหรัฐอเมริกา โดยในประเทศส่วนมาก อัตราคนที่จะเป็นโรคในช่วงชีวิตอยู่ระหว่าง 8-12% และงานสำรวจประจำชาติปี 2546 ของประเทศไทยแสดงความชุกที่ 3.2% (ประมาณ 871,700 คน)

จำนวนปีที่เสียไปเนื่องจากปัญหาสุขภาพ ความพิการ หรือการเสียชีวิตก่อนอายุ (DALY) ที่ปรับมาตรฐานตามอายุ (Age-standardised) ของโรคซึมเศร้าแบบขั้วเดียว (unipolar) ในประเทศต่าง ๆ (ต่อประชากร 100,000 คน) ในปี 2547 ในประเทศไทย เป็นโรคที่มี DALY ในระดับท๊อป 10 โดยเป็นอันดับ 1 ในหญิง และอันดับ 4 ในชาย

ในอเมริกาเหนือ โอกาสที่จะมีคราวซึมเศร้า (major depressive episode) ในช่วงระยะเวลาปีหนึ่งอยู่ที่ 3-5% สำหรับชาย และ 8-10% สำหรับหญิง

พลศาสตร์ประชากร

งานศึกษากลุ่มประชากรพบอย่างสม่ำเสมอว่า โรคเกิดขึ้นในหญิง 2 เท่ามากกว่าชาย แม้ว่าจะไม่ชัดเจนว่าเพราะอะไร หรือว่ามีปัจจัยอะไรอื่นที่เป็นเหตุความแตกต่างนี้หรือไม่ ในเด็ก การเกิดโรคเพิ่มขึ้นตามพัฒนาการของวัยเจริญพันธุ์ไม่ใช่ตามอายุ และจะถึงอัตราระดับผู้ใหญ่ในช่วงอายุ 15-18 ปี และดูเหมือนจะสัมพันธ์กับปัจจัยทางจิต-สังคมมากกว่าปัจจัยทางฮอร์โมน

คนมีโอกาสที่จะประสบกับคราวซึมเศร้าในระหว่างอายุ 30-40 ปีมากที่สุด และมีช่วงการเกิดสูงสุดแต่ต่ำลงมาในช่วงอายุ 50-60 ปี ความเสี่ยงต่อโรคสูงขึ้นเมื่อมีปัญหาทางประสาทอื่น ๆ เช่น โรคหลอดเลือดสมอง โรคพาร์คินสัน หรือโรคปลอกประสาทเสื่อมแข็ง และปีแรกหลังจากคลอดบุตร ความเสี่ยงโรคสัมพันธ์กับตัวก่อความเครียดที่คนทั้งกลุ่มเผชิญด้วย เช่น ผู้ต่อสู้ในสงครามหรือแพทย์ฝึกหัด

โรคสามัญกว่าด้วยหลังเกิดความเจ็บป่วยเกี่ยวกับโรคหลอดเลือดหัวใจ และสัมพันธ์กับเมื่อฟื้นตัวได้ไม่ดีมากกว่าเมื่อฟื้นตัวดีกว่า งานศึกษาแสดงผลที่ขัดแย้งกันเกี่ยวกับความชุกของโรคในคนชรา แต่ข้อมูลโดยมากแสดงว่าระดับลดลงในคนวัยนี้ โรคสามัญในเขตเมืองมากกว่าเขตชนบท และสามัญในกลุ่มที่มีปัญหาทางสังคม-เศรษฐกิจมากกว่า เช่น ในคนจรจัด

ข้อมูลในเรื่องความชุกสัมพัทธ์ของโรคระหว่างคนชาติพันธุ์ต่าง ๆ ยังไม่มีความเห็นพ้องทางวิทยาศาสตร์ แต่ว่า งานเดียวที่ศึกษา dysthymia โดยเฉพาะพบว่า โรคสามัญในกลุ่มชาวอเมริกันเชื้อสายแอฟริกาและเชื้อสายเม็กซิกัน มากกว่าคนอเมริกันเชื้อสายยุโรป

มีงานศึกษาที่คาดการณ์ว่า โรคซึมเศร้าจะเป็นโรคที่ทำให้เสียชีวิตเป็นอันดับสองต่อจากโรคหัวใจทั่วโลกโดยปี 2563
