วัคซีนนี้เป็นวัคซีนเวกเตอร์ไวรัสคือใช้ไวรัสเป็นตัวนำพา ไวรัสที่ใช้เป็นอะดีโนไวรัสประจำในมนุษย์ที่ได้ดัดแปลงพันธุกรรมให้มียีนเพื่อสร้างโปรตีนหนาม (spike protein) ของไวรัสโควิด-19 (SARS-CoV-2)ระบบภูมิคุ้มกันของร่างกายจะตอบสนองต่อโปรตีนหนามนี้โดยสร้างสารภูมิต้านทาน (antibodies) วัคซีนต้องฉีดแค่เข็มเดียวและไม่จำเป็นต้องเก็บแช่แข็ง

การทดลองทางคลินิกของวัคซีนนี้ได้เริ่มในเดือนมิถุนายน 2020 การทดลองระยะที่ 3 ของวัคซีนมีอาสาสมัครราว ๆ 43,000 คน ในวันที่ 28 มกราคม 2021 แยนส์เซนประกาศผลการทดลองว่า เริ่มตั้งแต่วันที่ 28 หลังฉีด วัคซีนโดสหนึ่งมีประสิทธิศักย์ป้องกันการติดเชื้อโควิด-19 อันแสดงอาการที่อัตราร้อยละ 66, ป้องกันการติดเชื้อที่มีอาการรุนแรงร้อยละ 85 และป้องกันการติดเชื้อที่ต้องเข้า รพ. หรือที่ถึงชีวิตได้เต็มร้อย

องค์การอาหารและยาสหรัฐอนุมัติให้ใช้วัคซีนเป็นการฉุกเฉิน สำนักงานการแพทย์ยุโรป (European Medicines Agency, EMA) ได้อนุมัติให้วางตลาดอย่างมีเงื่อนไข

คณะกรรมการอาหารและยาไทย (อย.) ได้ขึ้นทะเบียนวัคซีนนี้แล้ว รัฐบาลไทยกำลังดำเนินการจัดหาให้ได้วัคซีน 5-10 ล้านโดสในปี 2021 เพื่อให้ครบเป้าหมายในการฉีดวัคซีนโควิด-19 ให้ประชาชน 100 ล้านโดส โดยกรมควบคุมโรคคาดว่าอาจจะได้จัดสรรเริ่มในเดือนกันยายน

การแพทย์

วัคซีนนี้ใช้ป้องกันการติดเชื้อไวรัสโควิด-19 (SARS-CoV-2) เพื่อไม่ให้ติดโรคสำหรับผู้มีอายุ 18 ปีหรือยิ่งกว่า

รายละเอียด

วัคซีนนี้ใช้เทคโนโลยีคล้าย ๆ กับวัคซีนสปุตนิกวีและวัคซีนโควิด-19 ของออกซฟอร์ด-แอสตราเซเนกา แต่เพราะเป็นวัคซีนอะดีโนไวรัสของมนุษย์ จึงทำให้วัคซีนนี้คล้ายกับสปุตนิกวียิ่งกว่า วัคซีนมีองค์ประกอบออกฤทธิ์เป็นอะดีโนไวรัสลูกผสม (recombinant) ที่ไม่สามารถถ่ายแบบยีนได้ (replication-incompetent) ชนิดที่ 26 (Ad26) ซึ่งสามารถแสดงออกโปรตีนหนาม (โปรตีนเอส) ของไวรัสโควิด-19 (severe acute respiratory syndrome coronavirus 2, SARS-CoV-2) โดยอยู่ในโครงรูปที่เสถียร (stabilized conformation) โปรตีนหนามโครงสรูปเสถียรนี้ ซึ่งมีจุดกลายพันธุ์สองจุดที่กรดอะมิโนธรรมดาได้แทนที่ด้วย proline ได้พัฒนาขึ้นโดยนักวิจัยที่ศูนย์วิจัยวัคซีนของสถาบันภูมิแพ้และโรคติดเชื้อแห่งชาติสหรัฐ (National Institute of Allergy and Infectious Diseases, NIAID) โดยร่วมมือกับมหาวิทยาลัยเท็กซัส ออสติน วัคซีนมีส่วนประกอบไม่ออกฤทธิ์ดังต่อไปนี้คือ citric acid monohydrate, trisodium citrate dihydrate, เอทานอล, 2-hydroxypropyl-β-cyclodextrin (HBCD) (hydroxypropyl betadex), polysorbate 80, โซเดียมคลอไรด์, โซดาไฟ และกรดไฮโดรคลอริก

ลักษณะต่าง ๆ

วัคซีนนี้ฉีดเพียงเข็มเดียว ไม่ต้องฉีดเข็มที่สอง ซึ่งไม่เหมือนกับวัคซีนโควิด-19 อื่น ๆ (เช่น วัคซีนของออกซฟอร์ด-แอสตราเซเนกา ของไฟเซอร์-ไบออนเทค หรือของโมเดอร์นา)

ขวดวัคซีนที่ยังไม่เจาะใช้สามารถเก็บในอุณหภูมิระหว่าง 9-25 องศาเซลเซียสได้ 12 ชม. และเก็บไว้ในตู้เย็นธรรมดาได้เป็นเดือน ๆ วัคซีนไม่ต้องส่ง หรือเก็บแช่แข็ง

ผลที่ไม่พึงประสงค์

ในการทดลอง ผลข้างเคียงของวัคซีนที่พบปกติจะมีอาการเบาจนถึงปานกลาง เกิดในสองวันหลังฉีดวัคซีน และดีขึ้นเองภายใน 1-2 วัน

การไอ ปวดข้อ มีไข้ หนาวสะท้าน บวมแดงที่จุดฉีดเกิดกับคนน้อยกว่า 1 ใน 10การจาม สั่น เจ็บคอ เป็นผื่น เหงื่ออก กล้ามเนื้ออ่อนแรง เจ็บปวดแขนหรือขา ปวดหลัง เพลีย และรู้สึกไม่สบายเกิดกับคนน้อยกว่า 1 ใน 100 ผลข้างเคียงที่มีน้อย (คือเกิดกับคนน้อยกว่า 1 ในพัน) ก็คือการแพ้และผื่นคัน การเกิดลิ่มเลือดในเส้นเลือด (thrombosis) พร้อมกับภาวะเกล็ดเลือดน้อยเกิดขึ้นกับคนน้อยกว่า 1 ในหมื่นใน 1-2 สัปดาห์หลังฉีดวัคซีน อาการแพ้รวมทั้งแอนาฟิแล็กซิสคือการแพ้รุนแรง เกิดขึ้นในกรณีน้อย (rare) ภายไม่กี่นาทีจนถึงหนึ่ง ชม. หลังได้วัคซีน ควรฉีดวัคซีนในการดูแลของแพทย์พยาบาลที่สามารถช่วยเหลือได้ทันถ้าเกิดอาการรุนแรง

การพัฒนา

ในช่วงการระบาดทั่วของโควิด-19 บริษัทจอห์นสันแอนด์จอห์นสันได้ลงทุนเกินกว่า 1,000 ล้านดอลลาร์สหรัฐ (ประมาณ 31,300 ล้านบาท) เพื่อพัฒนาวัคซีนโดยร่วมมือกับองค์กรต่าง ๆ ของรัฐบาลกลางสหรัฐรวมท้งสำนักงานการวิจัยและพัฒนาชีวเวชภัณฑ์ชั้นสูง (Biomedical Advanced Research and Development Authority, BARDA) และกระทรวงสาธารณสุขและบริการประชาชนของสหรัฐอเมริกา (Department of Health and Human Services, HHS) บริษัทตั้งใจจะดำเนินการพัฒนาวัคซีนอย่างไม่ได้หวังกำไรเพราะมองว่า เป็นวิธีที่เร็วและดีที่สุดเพื่อให้ได้ความร่วมมือในการผลิตวัคซีนได้เป็นจำนวนมาก ๆ

แยนส์เซนวัคซีนได้ร่วมมือกับศูนย์การเแพทย์อิสราเอล (Beth Israel Deaconess Medical Center, BIDMC) เพื่อพัฒนาวัคซีนแคนดิเดนโดยใช้เทคโนโลยีเดียวกับที่สร้างวัคซีนอีโบลาของบริษัท

การทดลองพรีคลินิกระบุว่า วัคซีนมีประสิทธิภาพป้องกันแฮมสเตอร์และลิงวอกจากเชื้อโควิด-19

ระยะ 1-2

ในเดือนมิถุนายน 2020 จอห์นสันแอนด์จอห์นสันกับสถาบันภูมิแพ้และโรคติดเชื้อแห่งชาติสหรัฐ (NIAID) ได้ยืนยันว่ามีแผนทดลองทางคลินิกสำหรับวัคซีน Ad26.COV2.S โดยอาจเริ่มอย่างเร่งด่วนในปลายเดือนกรกฎาคม

การทดลองทางคลินิกระยะที่ 1-2a ได้เริ่มกับอาสาสมัครคนแรก ณ วันที่ 15 กรกฎาคม 2020 ในประเทศเบลเยียมและสหรัฐ ผลงานศึกษาในระหว่างแสดงว่า วัคซีนปลอดภัย มีผลข้างเคียงไม่เกินปกติ และก่อการตอบสนองทางภูมิคุ้มกัน การฉีดวัคซีนครั้งเดียวทำให้อาสาสมัครเกินร้อยละ 90 เกิดสารภูมิต้านทานพอกำจัดไวรัสหลังจากได้วัคซีน 29 วัน โดยเพิ่มเป็นเต็มร้อยหลัง 57 วัน

ระยะ 3

การทดลองทางคลินิกระยะ 3 ที่ตั้งชื่อว่า ENSEMBLE ได้เริ่มรับอาสาสมัครเมื่อเดือนกันยายน 2020 และหยุดรับอาสาสามัครเมื่อเดือนธันวาคม 2020 เป็นงานศึกษาแบบสุ่ม มีกลุ่มควบคุมที่ใช้ยาหลอก อำพรางทั้งสองฝ่าย เพื่อตรวจสอบความปลอดภัยและประสิทธิศักย์ ของวัคซีนโดสเดียวเทียบกับยาหลอกในผู้ใหญ่อายุ 18 ปีหรือยิ่งกว่า อาสาสมัครได้รับวัคซีนโดสเดียวที่มีอนุภาคไวรัส (virus particle) 5×1010 อนุภาคในวันแรกโดยฉีดเข้ากล้ามเนื้อ การทดลองได้หยุดชั่วคราวในวันที่ 12 ตุลาคม 2020 เพราะอาสาสมัครคนหนึ่งเกิดป่วย แต่บริษัทระบุว่า ไม่พบหลักฐานใด ๆ ว่าวัคซีนเป็นเหตุให้ป่วยและประกาศในวันที่ 23 ตุลาคมว่าจะดำเนินการทดลองต่อ ในวันที่ 29 มกราคม 2021 แยนส์เซนประกาศข้อมูลความปลอดภัยและประสิทธิศัยก์ที่ได้จากการวิเคราะห์การทดลองในระหว่าง ซึ่งแสดงว่าวัคซีนมีประสิทธิศักย์ป้องกันการติดเชื้อที่มีอาการปานกลางจนถึงรุนแรงหลังฉีดวัคซีน 28 วันที่อัตราร้อยละ 66 สำหรับอาสาสมัครทั้งหมด ผลได้จากการวิเคราะห์การติดโรคแบบแสดงอาการ 468 คนจากอาสาสมัครผู้ใหญ่รวม 43,783 คนในประเทศอาร์เจนตินา บราซิล ชิลี โคลอมเบีย เม็กซิโก เปรู แอฟริกาใต้ และสหรัฐอเมริกา กลุ่มที่ได้วัคซีนไม่มีใครเสียชีวิตเพราะโควิด แต่กลุ่มยาหลอกมีคนเสียชีวิตเนื่องกับโควิด 5 ราย ในการทดลองนี้ ไม่พบอาการแพ้วัคซีนอย่างรุนแรง (แอนาฟิแล็กซิส)

มีการทดลองทางคลินิกระยะที่ 3 อีกงานซึ่งตั้งชื่อว่า ENSEMBLE 2 และได้เริ่มรับสมัครอาสาสมัครในวันที่ 12 พฤศจิกายน 2020 โดยต่างกับ ENSEMBLE งานแรกเพราะอาสาสมัครจะได้วัคซีน 2 โดส โดสแรกในวันแรกและโดสที่สองในวันที่ 57

การผลิต

ในเดือนเมษายน 2020 จอห์นสันแอนด์จอห์นสันเริ่มทำงานร่วมกับบริษัท Catalent เพื่อผลิตวัคซีนเป็นจำนวนมาก ๆ ที่โรงงานของ Catalent ที่รัฐอินดีแอนา ในเดือนกรกฎาคม 2020 ก็ได้เพิ่มโรงงานของ Catalent อีกแห่งในอิตาลี

ในเดือนกรกฎาคม 2020 จอห์นสันแอนด์จอห์นสันตกลงส่งวัคซีนให้สหรัฐ 100 ล้านโดส มีมูลค่าเกิน 1,000 ล้านดอลลาร์สหรัฐ (ประมาณ 31,300 ล้านบาท) โดยเป็นงบประมาณจากจากสำนักงานการวิจัยและการพัฒนาชีวเวชภัณฑ์ชั้นสูงสหรัฐ (BARDA) และกระทรวงสาธารณสุขและบริการประชาชนของสหรัฐอเมริกา (Department of Health and Human Services, HHS)

ในเดือนธันวาคม 2020 บริษัทตกลงให้บริษัทสเปน Reig Jofre ผลิตวัคซีนที่โรงงานในเมืองบาร์เซโลนา ถ้าสำนักงานการแพทย์ยุโรป (European Medicines Agency, EMA) ให้อนุมัติวัคซีนในเดือนมีนาคม 2021 เจ้าหน้าที่สหภาพยุโรประบุว่า J & J อาจส่งวัคซีนให้ประเทศสหภาพยุโรปเริ่มตั้งแต่เดือนเมษายน 2021 เริ่มตั้งแต่ไตรมาสที่ 2 ของปี 2021 โรงงานจะผลิตวัคซีนโควิด-19 เป็นจำนวน 50 ล้านโดสต่อปี

ในเดือนกุมภาพันธ์ 2021 J & J ตกลงให้บริษัทซาโนฟี่ผลิตวัคซีนในโรงงานที่ประเทศฝรั่งเศส โดยจะผลิตประมาณ 12 ล้านโดสต่อเดือนเมื่อได้อนุมัติ

ในเดือนมีนาคม 2021 J & J ตกลงให้เมอร์คผลิตวัคซีนที่โรงงานสองแห่งในสหรัฐโดยทำตามกฎหมายการผลิตทางกลาโหมสหรัฐ (Defense Production Act) ในเดือนเดียวกัน ความผิดพลาดของบุคลากรในโรงงานของ Emergent BioSolutions ในเมืองบอลทิมอร์ก็ทำให้วัคซีนของบริษัทเสียไปอาจถึง 15 ล้านโดส ซึ่งพบก่อนส่งวัคซีนออกจากโรงงาน แต่ก็ทำให้แผนการส่งวัคซีนในสหรัฐของบริษัทต้องล่าช้าลง ความผิดพลาดเป็นการผสมองค์ประกอบวัคซีนของ J & J กับของแอสตราเซเนกาเข้าด้วยกัน ต่อมารัฐบาลกลางสหรัฐต่อมาจึงมอบโรงงานนี้ให้แก่ J & J เพื่อจัดการผลิตวัคซีนของบริษัทเท่านั้น และไม่ให้มีเหตุการณ์ผสมวัคซีนกันเช่นนี้อีก

สหภาพยุโรป

เริ่มตั้งแต่วันที่ 1 ธันวาคม 2020 คณะกรรมการเวชภัณฑ์สำหรับมนุษย์ (Committee for Medicinal Products for Human Use) ของสำนักงานการแพทย์ยุโรป (EMA) ได้เริ่มตรวจผลการทดลองทางคลินิกของวัคซีนนี้โดยทำอยู่เรื่อย ๆ เพื่อเร่งกระบวนการให้อนุมัติการวางตลาดของวัคซีน ในวันที่ 26 กุมภาพันธ์ 2021 แยนส์เซนก็ได้ยื่นคำขออนุมัติการวางตลาดอย่างมีเงื่อนไข ซึ่งคณะกรรมาธิการยุโรปก็ได้อนุมัติ (โดยระบุชื่อวัคซีนว่า COVID-19 Vaccine Janssen) เมื่อวันที่ 11 มีนาคม 2021 การส่งวัคซีนจะเริ่มในปลายเดือนเมษายน โดยบริษัทมีข้อผูกมัดว่าจะต้องส่ง 200 ล้านโดสให้แก่สหภาพยุโรปในปี 2021

ต่อมาวันที่ 20 เมษายน การแจกจำหน่ายวัคซีนนี้ในยุโรปก็ถูกชะลอไปเล็กน้อยจนกระทั่งสำนักงานการแพทย์ยุโรปได้ตัดสินว่า การช่วยระงับเหตุการณ์ระบาดทั่วของวัคซีนมีประโยชน์ยิ่งกว่าปัญหาการเกิดลิ่มเลือดเนื่องจากวัคซีนที่มีน้อย

สหรัฐ

ในวันที่ 4 กุมภาพันธ์ 2021 แยนส์เซนได้ยื่นคำขออนุมัติการใช้วัคซีนเป็นการฉุกเฉินแก่องค์การอาหารและยาสหรัฐ โดยองค์การก็ประกาศว่า ผู้เชี่ยวชาญจะประชุมพิจารณาในวันที่ 26 กุมภาพันธ์ และบริษัทก็ระบุว่า จะส่งวัคซีนทันทีที่ได้อนุมัติ ต่อมาวันที่ 24 องค์การได้ตีพิมพ์เอกสารที่แนะนำให้อนุมัติ โดยสรุปว่า ผลของการทดลองทางคลินิกและข้อมูลด้านความปลอดภัยเข้าเกณฑ์ขององค์การและของสำนักงานการแพทย์ยุโรป ในวันที่ 26 คณะผู้เชี่ยวชาญก็ได้แนะนำอย่างเป็นเอกฉันท์ให้อนุมัติวัคซีนเพื่อใช้เป็นการฉุกเฉิน ซึ่งองค์การก็ให้อนุมัติในวันต่อมา ต่อมาวันที่ 26 กุมภาพันธ์ ศูนย์ควบคุมและป้องกันโรคสหรัฐจึงแนกนำให้ใช้วัคซีนกับคนผู้มีอายุ 18 ปีหรือยิ่งกว่า

ในวันที่ 13 เมษายน 2021 ศูนย์ควบคุมและป้องกันโรคและองค์การอาหารและยาร่วมกันแนะนำให้หยุดใช้วัคซีนชั่วคราว เพราะมีรายงานกรณี cerebral venous sinus thrombosis เป็นการเกิดลิ่มเลือดบวกกับการมีเกล็ดเลือดน้อยซึ่งมีน้อยแต่รุนแรงในหญิงที่ได้วัคซีนอายุระหว่าง 18-48 ปี อาการเกิดใน 6-13 วันหลังได้วัคซีน โดยมีหญิงคนหนึ่งเสียชีวิตและอีกคนหนึ่งมีอาการหนักอยู่ใน รพ.

ในวันที่ 23 เมษายน องค์กรทั้งสองก็ได้ตัดสินว่า ควรดำเนินฉีดวัคซีนนี้ต่อในสหรัฐ โดยมีการระบุเพิ่มในใบอนุมัติให้ใช้วัคซีนว่า มีโอกาสเสี่ยงอาการเช่นนี้ โดยเรียกว่า thrombosis-thrombocytopenia syndrome (TTS)

ที่อื่น ๆ

ในเดือนธันวาคม 2020 บริษัทได้ตกลงกับองค์การกาวีเพื่อส่งวัคซีนให้โคแวกซ์ ต่อมาวันที่ 19 กุมภาพันธ์ 2021 บริษัทจึงได้ยื่นคำขอพร้อมข้อมูลอย่างเป็นทางการแก่องค์การอนามัยโลกเพื่อให้วัคซีนเป็นส่วนในรายการเวชภัณฑ์ที่ใช้ได้เป้นการฉุกเฉิน (Emergency Use Listing, EUL) โดย EUL เป็นเกณฑ์ที่ต้องผ่านก่อนจะส่งวัคซีนแก่โคแวกซ์ บริษัทคาดว่า จะส่งวัควีนให้แก่โคแวกซ์ทั้งหมด 500 ล้านโดสตลอดจนถึงปี 2022

ในปลายเดือนพฤศจิกายน 2020 บริษัทยื่นคำขออนุมัติวัคซีนแก่กระทรวงสาธารณสุขแคนาดา รัฐบาลกลางแคนาดาก็ได้ส่งวัคซีน 10 ล้านโดสโดยมีสิทธิเลือกซื้ออีก 28 ล้านโดส ในวันที่ 5 มีนาคม วัคซีนนี้ได้ขึ้นทะเบียนเป็นวัคซีนโควิด-19 ชนิดที่ 4 ซึ่งอนุมัติให้ใช้ในแคนาดา

ในเดือนเมษายน 2021 รัฐบาลกลางออสเตรเลียระบุว่า จะไม่ส่งซื้อวัคซีนนี้ เพราะ รัฐบาล "ไม่ตั้งใจจะซื้อวัคซีนอะดีโนไวรัสเพิ่มขึ้นในตอนนี้"

ประเทศไทย

ในต้นเดือนกุมภาพันธ์ 2021 รองเลขาธิการคณะกรรมการอาหารและยา (อย.) ระบุว่า จอห์นสันแอนด์จอห์นสันในประเทศไทยได้ยื่นเอกสารเพื่อขอขึ้นทะเบียนวัคซีนโควิด-19 แล้วโดยการให้อนุมัติจะพิจารณาภายใน 30 วันหลังจากได้ยื่นเอกสารครบ ต่อมาปลายเดือน รมว. กระทรวงสาธารณสุขจึงระบุว่า อย. ได้อนุมัติขึ้นทะเบียนวัคซีนนี้แล้ว โดยเป็นวัคซีนโควิด-19 ที่ 3 ซึ่งได้อนุมัติในประเทศไทยต่อจากซิโนแว็กและแอสตราเซเนกา

รัฐบาลไทยกำลังดำเนินการจัดหาให้ได้วัคซีนของจอห์นสันแอนด์จอห์นสัน 5-10 ล้านโดสในปี 2021 เพื่อให้ครบเป้าหมายในการฉีดวัคซีนโควิด-19 ให้ประชาชน 100 ล้านโดส นี่เป็นวัคซีนนอกเหนือจากวัคซีนของแอสตราเซเนกาและของซิโนแวกที่ได้ฉีดให้แก่ประชาชนแล้ว บวกกับวัคซีนที่จะจัดหาเพิ่มอื่น ๆ รวมทั้งสปุตนิกวีและวัคซีนของไฟเซอร์ โดยกรมควบคุมโรคคาดว่าอาจจะได้จัดสรรเริ่มในเดือนกันยายน

การใช้และผล

เพราะวัคซีนนี้ต้องฉีดเพียงแค่โดสเดียวและมีราคาถูกกว่า จึงคาดว่าจะมีบทบาทสำคัญในประเทศที่มีรายได้น้อยจนถึงปานกลาง เพราะไม่จำเป็นต้องแช่แข็งเหมือนกับวัคซีนโควิด-19 ของไฟเซอร์และโมเดิร์นา จึงสามารถขนส่ง เก็บ และบริหารได้ง่ายกว่า แอฟริกาใต้ประกาศในวันที่ 9 กุมภาพันธ์ 2021 ว่าจะขายวัคซีนของแอสตราเซเนกา แล้วเปลี่ยนมาใช้วัคซีนนี้แทน แล้วก็เริ่มฉีดวัคซีนนี้แก่ประชาชนทั่วไปในวันที่ 17 กุมภาพันธ์ 2021 เป็นการฉีดวัคซีนครั้งแรกนอกการทดลองทางคลินิก
