โรคดิสโซสิเอทีฟ

โรคดิสโซสิเอทีฟ (อังกฤษ: Dissociative Disorders) เป็นภาวะที่ความทรงจำ สติสัมปชัญญะ ความรู้ตัว เอกลักษณ์ และ/หรือการรับรู้สภาพแวดล้อมของผู้ป่วยเสียไปหรือถูกรบกวน สมมติฐานบอกว่าอาการนี้เป็นผลจากการประสานงานกันของการทำงานดังกล่าวสูญเสียตั้งแต่หนึ่งอย่างขึ้นไป

จากเกณฑ์การวินิจฉัยโรคทางจิตเวช Diagnostic and Statistical Manual of Mental Disorders ฉบับปรับปรุงครั้งที่ 4 (DSM IV) มีโรคในกลุ่มนี้ได้แก่

  • โรคดีเพอร์ซันนอลไลเซชัน (อังกฤษ: Depersonalization disorder; รหัส DSM-IV 300.6) คือผู้ป่วยมีช่วงเวลาที่รู้สึกแยกออกจากร่างกายตนเองหรือสิ่งแวดล้อม ซึ่งมักรู้สึกว่าไม่ใช่สิ่งที่มีอยู่จริง (กล่าวคือขาดการควบคุมตนเองและภายนอกตนเอง) ในระหว่างนี้ผู้ป่วยยังสามารถรับรู้ว่าสิ่งที่เกิดเป็นเพียงความรู้สึกและไม่ได้เป็นความจริง
  • ภาวะลืมแบบดิสโซสิเอทีฟ (อังกฤษ: Dissociative amnesia; รหัส DSM-IV 300.12) หรือชื่อเดิมคือ Psychogenic Amnesia คือภาวะที่ความจำบกพร่องอันเนื่องจากการกระทบกระเทือนทางอารมณ์อย่างรุนแรง
  • ดิสโซสิเอทีฟ ฟิวจ์ (อังกฤษ: Dissociative fugue; รหัส DSM-IV 300.13) หรือชื่อเดิมคือ Psychogenic Fugue คือผู้ป่วยละทิ้งสิ่งแวดล้อมหรือประสบการณ์ที่คุ้นเคย รวมทั้งจำอดีตของตนเองไม่ได้ ภาวะนี้อาจเกิดการสับสนระหว่างเอกลักษณ์จริง ๆ ของตนกับเอกลักษณ์ใหม่ได้
  • บุคลิกภาพแตกแยก (อังกฤษ: Dissociative identity disorder; รหัส DSM-IV 300.14) หรือชื่อเดิมและชื่อที่ใช้ใน ICD-10 Multiple Personality Disorder คือผู้ป่วยมีบุคลิกภาพแตกต่างกันตั้งแต่ 2 บุคลิกขึ้นไปสลับเปลี่ยนกันมีบทบาทต่อพฤติกรรมโดยผู้ป่วยจะจำลักษณะข้อมูลสำคัญของอีกบุคลิกหนึ่งไม่ได้
  • โรคดิสโซสิเอทีฟ มิได้ระบุรายละเอียดเป็นอย่างอื่น (อังกฤษ: Dissociative disorder not otherwise specified; รหัส DSM-IV 300.15) ใช้เมื่อผู้ป่วยมีลักษณะพยาธิสภาพทางจิตเป็นแบบดิสโซสิเอชันซึ่งไม่สามารถอธิบายได้ด้วยโรคดิสโซสิเอทีฟที่จำเพาะอื่น ๆ
