ภาวะเลือดจางจากการสลายของเม็ดเลือดแดง หรือ โลหิตจางจากเม็ดเลือดแดงแตก (อังกฤษ: Hemolytic anemia) เป็นรูปแบบหนึ่งของภาวะเลือดจาง (anaemia) ที่เกิดจากการเฮโมไลซิส (hemolysis) การสลายตัวอย่างผิดปกติของเซลล์เม็ดเลือดแดง ไม่ว่าจะเป็นในหลอดเลือด (เฮโมไลซิสในหลอดเลือด; intravascular hemolysis) หรือที่อื่น ๆ ในร่างกายมนุษย์ (นอกหลอดเลือด; extravascular) โดยทั่วไปพบเกิดในม้าม แต่ในความเป็นจริงสามารถเกิดในอวัยวะอื่น ๆ ในระบบ reticuloendothelial system หรืออาจเกิดในลักษณะเชิงกล (mechanically) (ลิ้นพรอสเทติกได้รับการกระทบกระเทือน; prosthetic valve damage) ภาวะเลือดจางชนิดเฮโมไลติกคิดเป็น 5% ของภาวะเลือดจากทั้งหมดที่พบ ผลที่ตามมาจากภาวะเลือดจางแบบเฮโมไลติกมีหลายประการ ตั้งแต่อาการทั่วไปจนถึงอาการที่เป็นอันตรายแก่ชีวิต การจัดแบ่งโดยทั่วไปของภาวะเลือดจางแบบเฮโมไลติกนั้นแบ่งออกเป็นภายใน (intrinsic) หรือภายนอก (extrinsic) การรักษานั้นแตกต่างกันไป ขึ้นอยู่กับอาการและเหตุของโรค

อาการของภาวะเลือดจางแบบเฮโมไลติกนั้นเหมือนกับอาการของภาวะเลือดจางรูปแบบอื่น ๆ (อ่อนเพลีย, อาการหายใจลำบาก) แต่นอกจากนี้ การสลายตัวของเซลล์เม็ดเลือดแดงยังนำไปสู่ดีซ่าน และเพิ่มโอกาสเสี่ยงต่อการเกิดภาวะแทรกซ้อนในระยะยาว เช่น นิ่วในถุงน้ำดี และ ภาวะความดันหลอดเลือดปอดสูง

อาการ

อาการของภาวะเลือดจางแบบเฮโมไลติกนั้นเหมือนกับอาการทั่วไปของภาวะเลือดจาง เช่น อ่อนเพลีย, ตัวซีด, หายใจลำบาก และ หัวใจเต้นเร็วผิดปกติ ในเด็กเล็กอาจพบ ปัญหาการเจริญเติบโต (failure to thrive) ได้ในภาวะเลือดจางชนิดใด ๆ นอกจากนี้ อาการที่เกี่ยวข้องกับเฮโมไลซิสอาจพบได้ เช่น หนาวสั่น, ดีซ่าน, ปัสสาวะสีเข้ม และม้ามโต ประวัติทางการแพทย์ในบางมุมอาจนำไปสู่เหตุของการเกิดเฮโมไลซิสได้ เช่น ยา, โรคออโตอิมมูน (autoimmune disorders), ปฏิกิริยาเนื่องด้วยการถ่ายเลือด (blood transfusion), การมีลิ้นหัวใจเทียม เป็นต้น

เฮโมไลซิสเรื้อรัง (Chronic hemolysis) ทำให้เกิดการเพิ่มขึ้นของการขับบิลลิรูบินออกไปยังทางเดินน้ำดี ซึ่งอาจนำไปสู่การเกิดนิ่วในถุงน้ำดี การปล่อยเฮโมโกลบินอิสระอย่างต่อเนื่อง (continuous release of free hemoglobin) เกี่ยวพันกับการเกิดภาวะความดันหลอดเลือดปอดสูง (pulmonary hypertension; PH) ซึ่งภาวะนี้อาจนำไปสู่การวูบ (fainting), เจ็ลหน้าอก (chest pain) และอาการหอบแบบก้าวหน้า (progressive breathlessness) ภาวะความดันหลอดเลือดปอดสูง (pulmonary hypertension; PH) ในที่สุดจะนำไปสู่หัวใจห้องล่างขวาหัวใจล้มเหลว (right ventricular heart failure) ซึ่งมีอาการคือการบวมส่วนปลาย และอาการท้องมาน (ascites)
