ตับอักเสบ บี (อังกฤษ: hepatitis B) เป็นโรคตับชนิดหนึ่งที่เกิดจากการติดเชื้อไวรัสตับอักเสบบี ซึ่งอาจทำให้เกิดการติดเชื้อได้ทั้งแบบเฉียบพลันและแบบเรื้อรัง ผู้ป่วยที่ติดเชื้อครั้งแรกส่วนใหญ่ไม่มีอาการ บางรายจะมีอาการคลื่นไส้ อาเจียน ตัวเหลือง อ่อนเพลีย ปัสสาวะเข้ม และปวดท้องได้ ส่วนใหญ่อาการเหล่านี้จะเป็นอยู่ประมาณ 2-3 สัปดาห์ และมีเพียงส่วนน้อยที่อาการดำเนินไปอย่างรุนแรงจนถึงแก่ชีวิตในการติดเชื้อครั้งแรกนี้ ระยะฟักตัวอาจยาวนานได้ถึง 30-180 วัน ผู้ป่วยที่ติดเชื้อนี้ตั้งแต่แรกเกิดจะเกิดตับอักเสบเรื้อรังได้ถึง 90% ในขณะที่ผู้ป่วยที่ติดเชื้อในภายหลังที่อายุมากกว่า 5 ปี จะเกิดตับอักเสบเรื้อรังเพียง 10% ผู้ป่วยตับอักเสบเรื้อรังจากไวรัสตับอักเสบบีส่วนใหญ่จะไม่มีอาการอยู่เป็นเวลานาน แต่ในระยะท้าย ๆ อาจเกิดตับแข็งหรือมะเร็งตับขึ้นได้ ซึ่งภาวะแทรกซ้อนเหล่านี้เป็นสาเหตุการเสียชีวิตถึง 15-25% ของผู้ป่วยตับอักเสบเรื้อรังจากไวรัสบี

การติดต่อ

เชื้อไวรัสตับอักเสบ บี สามารถติดต่อทาง เลือด น้ำเชื้อ และน้ำหลั่งอย่างอื่น เช่น น้ำเหลือง ท่านสามารถรับเชื้อได้โดยวิธี

มีเพศสัมพันธ์กับคนที่มีเชื้อโดยไม่ได้สวมถุงยาง การจูบกันจะไม่ติดต่อถ้าปากไม่มีแผล ใช้เข็มฉีดยาร่วมกัน ใช้เข็มสักตามตัวหรือสีที่ใช้สักตามตัวร่วมกัน และการเจาะหู ใช้แปรงสีฟันร่วมกัน มีดโกน ที่ตัดเล็บ แม่ที่มีเชื้อสามารถติดต่อไปยังลูกได้ขณะคลอด ถ้าแม่มีเชื้อลูกมีโอกาสได้รับเชื้อ 90% และให้นมตัวเอง ถูกเข็มตำจากการทำงาน รักร่วมเพศกับผู้ที่มีเชื้ออยู่ โดยการสัมผัสกับ เลือด น้ำเลือด น้ำคัดหลั่ง โดยผ่านเข้าทางบาดแผล เชื้อนี้จะไม่ติดต่อกันทางลมหายใจ อาหาร หรือน้ำดื่ม การให้นม การจูบกัน

อาการ

การติดเชื้ออย่างเฉียบพลันของไวรัสตับอักเสบบีเกี่ยวโยงกับไวรัสตับอักเสบเฉียบพลัน ซึ่งเป็นโรคที่มีอาการตั้งแต่การป่วยโดยทั่วไป การไม่อยากอาหาร คลื่นไส้ อาเจียน ปวดตามร่างกาย ไข้อ่อน ๆ และปัสสาวะสีเข้ม จากนั้นอาการอาจพัฒนาเป็นโรคดีซ่าน โดยมีบันทึกว่าอาการคันเป็นหนึ่งในตัวชี้ถึงอาการของการติดเชื้อไวรัสตับอักเสบทุกสายพันธุ์ อาการป่วยจะคงอยู่ไม่กี่สัปดาห์และจากนั้นผู้ป่วยส่วนใหญ่จะมีอาการดีขึ้น มีเพียงไม่กี่คนที่อาการอาจพัฒนาสู่อาการตับวายและอาจเสียชีวิต การติดเชื้ออาจไม่แสดงอาการจึงทำให้ผู้ป่วยอาจไม่รู้ตัว

อาการอื่นที่ไม่เกี่ยวกับตับพบได้ใน 1-10% ของผู้ติดเชื้อไวรัสตับอักเสบ ได้แก่ โรค serum-sickness–like โรคโพลีอาเทอร์ไรติส โนโดซา (โรคแพน) เมมเบรนัสโกลเมอรูโลสเคลอโรซิส (membranous glomerulonephritis) และ กลุ่มอาการจานอตตี–กรอสตี โรค serum-sickness–like ที่เกิดจากไวรัสตับอักเสบบีมักตามด้วยโรคดีซ่าน อาการได้แก่มีไข้ ผื่นบนผิวหนัง และโรคโพลีอาเทอร์ไรติส โนโดซา ประมาณ 30–50% ของคนที่มีอาการโรคโพลีอาเทอร์ไรติส โนโดซาเป็นพาหะของไวรัสตับอักเสบ โรคไตที่มาจากไวรัสตับอักเสบพบได้ในผู้ใหญ่บางส่วน ทว่าส่วนใหญ่แล้วพบในเด็ก

การวินิจฉัย

การรักษา

การติดเชื้อตับอักเสบบีส่วนใหญ่ไม่จำเป็นต้องได้รับการรักษา เนื่องจากผู้ติดเชื้อแทบทั้งหมดสามารถกำจัดเชื้อได้เอง มีไม่ถึง 1% ที่อาจต้องได้รับการรักษาในระยะแรกด้วยยาต้านไวรัส ในกรณีที่ผู้ป่วยมีอาการรุนแรง (ตับอักเสบเต็มขั้น) หรือมีภูมิคุ้มกันบกพร่อง แต่ในทางกลับกันบางครั้งก็จำเป็นต้องให้ยาต้านไวรัสกับผู้ป่วยตับอักเสบบีเรื้อรัง เพื่อลดโอกาสการเกิดตับแข็งและมะเร็งตับ ผู้ป่วยกลุ่มที่ควรได้รับการรักษาด้วยยาต้านไวรัส เช่น ผู้ติดเชื้อเรื้อรังที่มีระดับเอนไซม์ ALT สูงตลอด และตรวจพบดีเอ็นเอของไวรัสตับอักเสบบี เป็นต้น ระยะในการรักษาอยู่ที่ 6 เดือน ถึง 1 ปี แล้วแต่ชนิดยาและพันธุกรรมของผู้ป่วย
