ความซึมเศร้า หรือ อารมณ์ซึมเศร้า (อังกฤษ: Depression) เป็นสภาวะอารมณ์หดหู่และไม่ชอบทำอะไร ๆ ที่อาจมีผลต่อความคิด พฤติกรรม ความรู้สึก และความเป็นอยู่ที่ดีของบุคคล

คนซึมเศร้าอาจรู้สึกเศร้า วิตกกังวล ไร้ความหมาย สิ้นหวัง ไม่มีที่พึ่ง ไม่ภูมิใจในตนเอง/ไม่มีค่า รู้สึกผิด หงุดหงิด โกรธ อับอาย หรือกระวนกระวาย อาจจะสูญความสนใจในกิจกรรมที่เคยชอบ ไม่อยากอาหารหรือทานมากเกินไป ไม่มีสมาธิ คอยระลึกถึงรายละเอียดในการตัดสินใจอะไรบางอย่าง มีปัญหาทางความสัมพันธ์ และอาจคิด พยายาม และทำการฆ่าตัวตาย การนอนไม่หลับ การนอนมากเกินไป อ่อนเปลี้ย เจ็บปวด มีปัญหาย่อยอาหาร และมีกำลังน้อยลง ก็อาจเป็นอาการร่วมด้วย

อารมณ์ซึมเศร้าเป็นลักษณะอาการทางจิตเวชบางอย่าง เช่น โรคซึมเศร้า (major depressive disorder) แต่ก็อาจะเป็นปฏิกิริยาปกติต่อเหตุการณ์ในชีวิต เช่น สูญเสียคนรัก ถ้ายังคงเป็นในระยะยาว และอาจเป็นอาการเจ็บป่วยทางกาย หรือเป็นผลข้างเคียงของยาหรือการรักษาทางแพทย์บางอย่าง

เหตุ

เหตุการณ์ในชีวิต

เหตุการณ์ร้ายในวัยเด็ก เช่น การสูญเสียคนรัก การถูกทอดทิ้ง การถูกทารุณกรรมทางกายหรือทางใจ ทารุณกรรมทางเพศ หรือการปฏิบัติไม่เท่าเทียมกันของพ่อแม่ระหว่างพี่น้อง อาจมีผลเป็นอารมณ์ซึมเศร้าในวัยผู้ใหญ่ ทารุณกรรมทางกายและทารุณกรรมทางเพศเป็นพิเศษ ถ้าไม่จัดการ จะเพิ่มโอกาสประสบความซึมเศร้าในช่วงชีวิต

การรักษาทางการแพทย์

มียาบางอย่างที่รู้ว่าเป็นเหตุของอารมณ์ซึมเศร้าในคนไข้เป็นจำนวนสำคัญ รวมทั้งยาสำหรับไวรัสตับอักเสบชนิดซี (เช่น อินเตอร์เฟียรอน) ยาแก้วิตกกังวล และยานอนหลับ (เช่นยากลุ่มเบ็นโซไดอาเซพีน เช่น alprazolam, คโลนะเซแพม, lorazepam และ diazepam) ยากันความดันสูง (เช่น เบต้า บล็อกเกอร์, methyldopa, และ reserpine) และการรักษาด้วยฮอร์โมน (เช่น corticosteroid และยาคุมกำเนิด) เป็นเรื่องสำคัญที่จะพิจารณาปัจจัยเหล่านี้เมื่อรักษาโรคซึมเศร้า

สารทำให้เกิด

อาการทางจิตเวชจำนวนหนึ่งมีอารมณ์ซึมเศร้าเป็นหลักอย่างหนึ่ง โดยมีความผิดปกติทางอารมณ์เป็นกลุ่มโรคที่มีปัญหาในด้านอารมณ์เป็นหลัก ซึ่งรวมทั้งโรคซึมเศร้า (MDD) ที่บุคคลมีอารมณ์ซึมเศร้าอย่างน้อยเป็น 2 อาทิตย์และสูญเสียความยินดีในกิจกรรมเกือบทั้งหมด และโรค dysthymia ซึ่งเป็นอารมณ์ซึมเศร้าเรื้อรัง ที่ไม่ถึงเกณฑ์วินิยฉัยของโรคซึมเศร้า ความผิดปกติทางอารมณ์อีกอย่างหนึ่งคือโรคอารมณ์สองขั้ว (bipolar disorder) ที่มีระยะ/คราวอารมณ์ครึกครื้น มีระดับการรู้คิดและพลังสูง แต่ก็อาจสลับกับคราวซึมเศร้า และถ้าโรคดำเนินไปตามฤดู โรคอาจจะเรียกว่าเป็นความผิดปกติทางอารมณ์ตามฤดู (seasonal affective disorder)

นอกจากกลุ่มความผิดปกติทางอารมณ์แล้วความผิดปกติทางบุคลิกภาพแบบก้ำกึ่ง (Borderline personality disorder) บ่อยครั้งมีอารมณ์ซึมเศร้าที่รุนแรง ส่วนความผิดปกติในการปรับตัวที่มีอารมณ์ซึมเศร้า (adjustment disorder with depressed mood) เป็นปัญหาทางอารมณ์ที่ปรากฏเป็นการตอบสนองต่อเหตุการณ์หรือตัวสร้างความเครียดที่ระบุได้ ซึ่งอาการทางอารมณ์และพฤติกรรมที่เป็นผลอยู่ในระดับสำคัญแต่ไม่ผ่านเกณฑ์วินิจฉัยของโรคซึมเศร้า:355 และความผิดปกติที่เกิดหลังความเครียดที่สะเทือนใจ (PTSD) ซึ่งเป็นความผิดปกติแบบวิตกกังวลอย่างหนึ่งที่บางครั้งติดตามความบาดเจ็บทางใจ และเกิดร่วมกับอารมณ์ซึมเศร้าอย่างสามัญ นอกจากนั้นแล้ว ความซึมเศร้าบางครั้งยังสัมพันธ์กับความผิดปกติเพราะการใช้สาร (substance use disorder) โดยยาทั้งถูกกฎหมายและผิดกฎหมายล้วนแต่สามารถเป็นเหตุของโรค

รอยด่างทางมรดก

นักวิจัยได้เริ่มพิจารณากระบวนการที่รอยด่างทางประวัติศาสตร์ในเรื่องการเดียดฉันท์โดยผิวพรรณ หรือการล่าอาณานิคม อาจสร้างเหตุปัจจัยให้เกิดความซึมเศร้า

ปัญหาทางเชื้อชาติ

มีความแตกต่างกันระหว่างเชื้อชาติเมื่อเผชิญหน้ากับความซึมเศร้าในสหรัฐอเมริกา ยกตัวอย่างเช่น ผู้ใหญ่ผิวดำมีโอกาสรายงานความทุกข์ที่สร้างปัญหา 20% มากกว่าผู้ใหญ่ผิวขาว นอกจากนั้นแล้ว ชายอเมริกันเชื้อสายแอฟริกายังมีปัญหาโรคเรื้อรังเช่นเบาหวานและมะเร็งในอัตราที่สูงกว่าชายผิวขาว และโรคหรือความผิดปกติเหล่านั้นเป็นปัจจัยเสี่ยงสำคัญต่อความซึมเศร้า

การประเมิน

ผู้ให้บริการทางสุขภาพจิตสามารถใช้แบบวัดความซึมเศร้าเช่น Beck Depression Inventory หรือ Children's Depression Inventory เพื่อช่วยตรวจจับ ความรุนแรงของความซึมเศร้า ยกตัวอย่างเช่น กรมสุขภาพจิต กระทรวงสาธารณสุขแนะแนวให้ผู้รักษาพยาบาลขั้นปฐมภูมิใช้แบบคำถาม 9 คำถามเป็นขั้นตอนในการวินิจฉัยโรคซึมเศร้า

การรักษา

อารมณ์ซึมเศร้าอาจไม่ต้องรักษาโดยมืออาชีพ เพราะอาจเป็นปฏิกิริยาปกติต่อเหตุการณ์ในชีวิต เป็นอาการของโรคอื่น ๆ หรือเป็นผลข้างเคียงของยาหรือการรักษาทางแพทย์ แต่ว่าอารมณ์ซึมเศร้าที่คงยืน โดยเฉพาะเมื่อมีกับอาการอื่น ๆ อาจจะเข้าเกณฑ์วินิจฉัยทางจิตเวชหรือทางแพทย์แบบต่าง ๆ ซึ่งสามารถได้ประโยชน์จากการรักษา โดยการรักษาโรคต่าง ๆ ในกลุ่มโรคซึมเศร้าจะแตกต่างกัน

ในสหรัฐอเมริกา ประเมินว่า คน 2/3 ที่มีโรคจะไม่หาวิธีรักษาองค์การอนามัยโลก (WHO) พยากรณ์ในปี 2551 ว่า โดยปี 2573 ความซึมเศร้าจะเป็นโรคที่ทำให้เกิดการพิการในระดับสูงสุดของโลกในบรรดาความผิดปกติทางกายและทางใจ

แนวทางการรักษาปี 2552 ขององค์การสาธารณสุขแห่งสหราชอาณาจักร (NICE) ชี้ว่า ยาแก้ซึมเศร้าไม่ควรใช้เป็นปกติเพื่อรักษาโรคซึมเศร้าขั้นอ่อน เพราะว่ามีอัตราความเสี่ยง-ประโยชน์ที่ไม่ดี การรักษาโรคจัดการได้ดีที่สุดโดยการเปลี่ยนรูปแบบชีวิต รวมทั้ง การทานอาหาร การหลับนอน และการออกกำลังกาย งานวิเคราะห์อภิมานปี 2559 แสดงว่า ยาแก้ซึมเศร้าโดยมากนอกจากฟลูอ๊อกซิติน ดูจะไม่มีประโยชน์ที่ชัดเจนเพื่อรักษาโรคซึมเศร้าในเด็กและวัยรุ่นแบบฉับพลัน

ความแตกต่างระหว่างเพศ

หญิงมีอัตราเป็นโรคซึมเศร้าที่สูงกว่าชาย หญิงมีอาการทางกายในระดับที่สูงกว่า เช่น ความเปลี่ยนแปลงในความอยากอาหาร การนอน และความอ่อนเปลี้ย ที่มีพร้อมกับความเจ็บปวดและความวิตกกังวล แต่ว่า ความแตกต่างระหว่างเพศในด้านอื่น ๆ อยู่ที่ระดับน้อยกว่ามาก
