ยาระบายอย่างอ่อน

ยาระบายอย่างอ่อน (อังกฤษ: laxative) หรือ ยาระบาย เป็นยาเพื่อลดความแน่นตัวของอุจจาระ และช่วยให้ถ่าย โดยสามารถใช้รักษาหรือป้องกันอาการท้องผูก ยาระบายต่าง ๆ ทำงานต่างกันและมีผลข้างเคียงต่างกัน ยาระบายแบบกระตุ้น (stimulant) แบบหล่อลื่น (lubricant) และแบบน้ำเกลืออาจใช้ชำระลำไส้ใหญ่ เพื่อให้แพทย์สามารถตรวจไส้ตรงและลำไส้ใหญ่ โดยอาจใช้ร่วมกับการสวนทวารในบางกรณี การทานยาระบายเป็นจำนวนมากอาจทำให้ท้องร่วง ยาบางอย่างอาจมีสารออกฤทธิ์มากกว่าตัวเดียว อาจใช้ทานหรือใช้เหน็บ

ประเภท

เพิ่มปริมาณอุจจาระ (Bulk-forming agents)

ยาระบายชนิดสร้างเนื้ออุจจาระมีองค์ประกอบที่เพิ่มเนื้อและน้ำในอุจจาระ เพื่อให้สามารถผ่านลำไส้ไปได้ง่ายขึ้น องค์ประกอบรวมทั้งใยอาหารและสารดูดน้ำ (hydrophilic agent)

คุณสมบัติ

  • ตำแหน่งออกฤทธิ์: ลำไส้เล็กและลำไส้ใหญ่
  • เวลาเริ่มออกฤทธิ์: 12-72 ชม.

ยาเพิ่มเนื้อจะดูดน้ำ ดังนั้นจึงควรทานพร้อมกับน้ำมาก ๆ เป็นยาเบาที่สุดในบรรดายาระบาย และสามารถใช้ป้องกันรักษาการขับถ่ายในระยะยาว

ใยอาหาร

อาหารที่ช่วยระบายก็คือที่มีใยอาหารสูง ทั้งแบบละลายน้ำไม่ได้และละลายได้ เช่น

ทำให้อุจจาระอ่อนนุ่ม (emollient agents, stool softeners)

ยาระบายแบบทำให้นุ่มและชุ่มชื้นควรทานพร้อมกับน้ำมาก ๆ เป็นยาที่ช่วยป้องกันท้องผูกมากกว่าเป็นยารักษาท้องผูกในระยะยาว

หล่อลื่น (Lubricant agents)

ยาระบายแบบหล่อลื่นช่วยเคลือบอุจจาระด้วยลิพิดลื่น ๆ และหน่วงการดูดซึมน้ำจากอุจจาระของลำไส้ใหญ่ จึงทำให้มันผ่านไปได้ง่ายกว่า อนึ่ง มันยังช่วยเพิ่มน้ำหนักและลดระยะเวลาที่ดำเนินผ่านลำไส้

แล็กทูโลสออกฤทธิ์โดยกระบวนการออสโมซิส ซึ่งเก็บน้ำไว้ในลำไส้ใหญ่ เพิ่มความเป็นกรดโดยผ่านกระบวนการหมักกลายเป็นกรดแล็กติก กรดฟอร์มิก และกรดน้ำส้ม แล้วเพิ่มการบีบตัว (peristalsis) ของลำไส้ใหญ่ ยาเหน็บคือ glycerin ออกฤทธิ์แบบ hyperosmotic คือเพิ่มปริมาณน้ำแล้วเพิ่มการบีบตัวของลำไส้ โดยยา sodium stearate ที่ผสมอยู่ด้วยกันก็จะระคายลำไส้ใหญ่ด้วย

ยาระบายแบบน้ำเกลือ

ยาระบายแบบกระตุ้น (stimulant laxative)

ยาระบายแบบกระตุ้นเป็นสารที่ออกฤทธิ์ที่เยื่อเมือกหรือข่ายประสาท (nerve plexus) ของลำไส้ เปลี่ยนแปลงการหลั่งน้ำและอิเล็กโทรไลต์ และยังกระตุ้นการบีบตัว (peristaltic) ของลำไส้ แต่อาจเป็นอันตรายภายใต้สถานการณ์บางอย่าง เช่นใช้เป็นระยะยาวซึ่งทำให้ปล้องลำไส้ (haustra) เปลี่ยนแปลงไปโดยคาดว่า เกิดความเสียหายต่อเซลล์ประสาทหรือกล้ามเนื้อ

คุณสมบัติ

นี่เป็นยาระบายที่มีฤทธิ์มากที่สุดและควรใช้อย่างระมัดระวัง การใช้ยาระบายแบบกระตุ้นนาน ๆ อาจทำให้ติดยาเพราะทำปล้องลำไส้ใหญ่ (haustral fold) ให้เสียหาย ทำให้ไม่สามารถเคลื่อนอุจจาระไปโดยไม่ใช้ยา งานศึกษาหนึ่งในคนไข้ที่ท้องผูกเรื้อรังพบว่า ผู้ใช้ยาระบายแบบกระตุ้น 28% จะเสียปล้องลำไส้ไปในระยะเวลา 1 ปี ในขณะที่กลุ่มควบคุมไม่เสีย

อื่น ๆ

น้ำมันละหุ่งเป็น glyceride ที่เอนไซม์จากตับอ่อนคือ pancreatic lipase จะสลายด้วยน้ำ (hydrolysis) ให้เป็นกรด ricinoleic ซึ่งมีฤทธิ์ระบายโดยยังไม่ทราบกลไก

ตัวทำการต่อหน่วยรับเซโรโทนิน (serotonin agonist)

มีสารกระตุ้นการบีบตัวของลำไส้โดยก่อสภาพกัมมันต์ของหน่วยรับเซโรโทนิน (5-HT4 receptor) ของระบบประสาทลำไส้ (enteric nervous system) ภายในทางเดินอาหาร แต่บางชนิดก็ได้เลิกใช้หรือจำกัดการใช้ เพราะโอกาสทำอันตรายต่อหัวใจหลอดเลือดโดยเป็นผลข้างเคียง tegaserod (ชื่อการค้า Zelnorm) ได้เลิกวางขายในตลาดทั่วไปในสหรัฐและแคนาดาในปี 2007 เนื่องจากมีรายงานว่าเพิ่มความเสี่ยงหัวใจวายและเส้นเลือดอุดตันในสมอง แต่ก็ยังมีให้แพทย์ใช้ในสถานการณ์ฉุกเฉินที่เป็นอันตรายต่อชีวิตหรือจำเป็นต้องใช้ในโรงพยาบาล

prucalopride (ยี่ห้อ Resolor) เป็นยาที่ปัจจุบันอนุมัติให้ใช้ในสหภาพยุโรปท้ายปี 2009 และในแคนาดา (ยี่ห้อ Resotran) ท้ายปี 2011 แต่ก็ยังไม่ได้อนุมัติให้ใช้ในสหรัฐอเมริกา โดยยังอยู่ใต้การพัฒนาของบริษัท Shire plc

สารก่อกัมมันต์ช่องคลอไรด์ (chloride channel activators)

lubiprostone เป็นยาที่ใช้รักษาท้องผูกเรื้อรังซึ่งไม่ทราบสาเหตุ (CIC) และกลุ่มอาการลำไส้ไวเกินต่อการกระตุ้น (IBS) โดยมีฤทธิ์ทำให้ลำไส้หลั่งน้ำที่สมบูรณ์ด้วยคลอไรด์ซึ่งทำอุจจาระให้นิ่ม เพิ่มการบีบตัวของลำไส้ และโปรโหมตการขับถ่ายอุจจาระแบบเกิดเอง (SBM)

ประสิทธิผล

สำหรับผู้ใหญ่ งานทดลองแบบสุ่มและมีกลุ่มควบคุมหนึ่งพบว่า polyethylene glycol (PEG) ยี่ห้อ MiraLax หรือ GlycoLax 17 กรัมวันละครั้งมีผลดีกว่า tegaserod 6 มก. 2 ครั้งต่อวัน อีกงานหนึ่งพบว่า PEG สองถุง (26 กรัม) มีผลดีกว่าแล็กทูโลสสองถุง (20 กรัม) การใช้ PEG วันละ 17 กรัมพบว่ามีประสิทธิผลและปลอดภัยในการทดลองแบบสุ่มและมีกลุ่มควบคุมเป็นเวลา 6 เดือน อีกงานหนึ่งพบว่า ซอร์บิทอลไม่มีผลต่างกับแล็กทูโลส

สำหรับเด็ก PEG พบว่ามีประสิทธิผลดีกว่าแล็กทูโลส

ปัญหาการใช้

การใช้ในทางที่ผิด

การใช้ยาระบายในทางที่ผิดมีผลลบต่าง ๆ ผลแบบเบารวมทั้งภาวะขาดน้ำ ความดันต่ำ หัวใจเต้นเร็ว เวียนศีรษะในบางอิริยาบถ (postural dizziness) และหมดสติชั่วคราว ผลแบบหนักที่อาจทำให้ถึงเสียชีวิตรวมทั้งการเสียดุลของความเป็นกรดด่างและของอิเล็กโทรไลต์ ยกตัวอย่างเช่น มีการสัมพันธ์ภาวะโพแทสเซียมต่ำในเลือดอย่างรุนแรงกับ distal renal tubular acidosis (ภาวะกระเดียดกรดเหตุหลอดไตฝอยส่วนปลาย) ที่เนื่องกับการใช้ยาระบายในทางที่ผิด โดยภาวะแอลคาโลซิสเนื่องกับเมแทบอลิซึม (metabolic alkalosis) เป็นภาวะไม่สมดุลของกรด-ด่างที่พบมากที่สุด ผลไม่พึงประสงค์ที่สำคัญอื่น ๆ รวมทั้ง rhabdomyolysis (ภาวะกล้ามเนื้อโครงร่างที่เสียหายสลายตัวเร็วเกิน) ภาวะไขมันเกินในอุจจาระ (steatorrhoea) เยื่อเมือกลำไส้ใหญ่อักเสบหรือเกิดแผลตับอ่อนอักเสบ, ไตวาย, คนไข้แกล้งมีท้องร่วง (factitious diarrhea) และปัญหาอื่น ๆ

แม้คนไข้ที่มีความผิดปกติในการรับประทาน เช่น โรคเบื่ออาหารเหตุจิตใจ (anorexia nervosa) และโรคทานแล้วขับออก (bulimia nervosa) บ่อยครั้งจะใช้ยาระบายในทางที่ผิดเพื่อพยายามลดน้ำหนัก แต่ยาระบายก็เพียงแต่เพิ่มความเร็วที่อุจจาระดำเนินผ่านลำไส้ใหญ่ ซึ่งเกิดหลังการดูดซึมอาหารในลำไส้เล็ก ดังนั้น งานศึกษาต่าง ๆ จึงได้แสดงว่า น้ำหนักที่ลดลงเพราะใช้ยาในทางที่ผิด โดยหลักมาจากการเสียน้ำจากร่างกายเพียงชั่วคราว ไม่ได้ลดพลังงานที่ดูดซึมเข้าร่างกายจริง ๆ

การชินต่อยาระบาย

แพทย์เตือนไม่ให้ใช้ยาระบายแบบกระตุ้นเป็นประจำ เนื่องจากปัญหาว่า อาจเป็นเหตุให้เนื้อเยื่อในลำไส้ใหญ่ทรุดโทรม แล้วทำให้ไม่สามารถขับอุจจาระได้เองเนื่องจากถูกกระตุ้นเป็นเวลานาน สิ่งสามัญที่พบในคนไข้ที่ใช้ยาระบายแบบกระตุ้นก็คือ ตะกอนสีน้ำตาลซึ่งสะสมที่เนื้อเยื้อลำไส้ ซึ่งเรียกว่า melanosis coli[ต้องการอ้างอิง]

การใช้ในประวัติศาสตร์เป็นต้น

ยาระบาย ซึ่งเคยเรียกในภาษาอังกฤษว่า physicks หรือ purgatives เป็นยาที่ใช้อย่างกว้างขวางก่อนการแพทย์ปัจจุบันเพื่อรักษาภาวะต่าง ๆ ภาวะซึ่งในปัจจุบันพิจารณาว่าเป็นยาที่ไม่ได้ผลตามเวชปฏิบัติอิงหลักฐาน ในนัยเดียวกัน ยาระบาย (โดยบางครั้งเรียกว่า การล้างลำไส้) ก็ยังได้การโปรโหมตจากแพทย์ทางเลือกในการรักษาภาวะต่าง ๆ รวมทั้งที่การแพทย์ปัจจุบันไม่ยอมรับ เช่น เพื่อกำจัดคราบ/ตะกอนอุจจาระในลำไส้ (mucoid plaque)
